Primary Operator Registration Instructions

INSTRUCTIONS

  1. Download and review the “CCL Guidelines 2017”. Find the applicable pathway according to your qualification and experience as described on page 5 and 6 of the guidelines as well as respective forms.
  2. Download and print the respective PSIC Operator Credentialing Form of applicable pathway.
  3. Fill up and sign the Credentialing form.
  4. Upload the filled & signed form along with other required documents in the “Application Submission” section below. Click “SUBMIT” button at the bottom to complete the process.
  5. Original filled form along with other documents to be sent by post to: Gen. Secretary PSIC, Office # 1602, High-Q tower, Gulberg-V Jail Road, Lahore. Phone number 042 32317355 (MANDATORY)
